Thank you for this. You and Dr. Faust continue to set a high standard for clear and thoughtful communication. I have found your work on Substack consistently helpful, especially in a time when so much information is either misleading or inaccessible.

While reading this, several additional thoughts came to mind. First, we now face a political climate where speaking clearly and publicly about science is not just challenging but risky. For those working in government or in institutions that rely on federal funding, there is a growing concern that simply stating scientific facts, particularly when they contradict the narratives promoted by Trump and his allies, can result in reprimands, firings, harassment, or even threats. Any serious training in science communication should include preparation for this kind of pressure, along with institutional strategies to support and protect those who speak up.

Second, even when high-quality information is available, much of it remains difficult to access. Some of the best Substacks and journal articles offer valuable insights, but access is often limited by paywalls or other barriers. Even the NEJM article, although free, requires creating an account. These may seem like minor hurdles, but they can prevent important information from reaching the broader public, especially those who are not already engaged or who have limited digital access. If we are serious about building public trust and improving health literacy, we need to remove these barriers wherever possible.

Well done! Simply interpreting the technical and foreign-sounding language of complex medical science into common language is a highly under-rated skill that deserves more attention in medical school.
