Thank you for this thoughtful review of what happened and what we could and should do better next time. During my emergency medicine career, I developed the practice of avoiding the use of the words "always" and "never". In trying to help my patients understand the probabilistic course of disease and treatment, I'd tell my patients that "always" and "never" are very big words that should rarely, if ever be used.
